Spruce, When this job was planned then all failures, Including the rope failing would wave been ID'd assessed and procedures put in place to lower the risk , they would have also identified the consequences of the failure (effects if you like) and also mitigated against those. Whether this was done using the traditional FMEA, FMEACA Bowtie RA's bespoke or more than likely a combination of all, who cares, but it would have been completed, risks identified (including rope failure) and procedures checked or put in place. - that is why the Operation would have been done using cameras only (no humans in the snap zone) no risk of damage to other critical equipment or to the environment and NO Major Incident Investigation required because there was no incident of this classification.
Unfortunately the rope failed, hopefully it doesn't next time and then the armchair experts can get their teeth into something else because personally the topic is starting to bore the tits off me.
