Luke, if you want to make plastic parts in high volume then you use a process called injection moulding, that requires tooling, this tooling is what the plastic is squirted into and hence is a perfectly detailed negative. Very detailed thing to make, hence very expensive. In this case I'd imagine that the channels that such the saliva to the test area probably have to be sized correctly to quite high tolerances, and certainly mating surfaces for the top and bottom need to be, else it would leak.
