I was doing a search regarding Scancell and esmo ... and came across something quite reassuring as a Scancell shareholder ... what if there was a study out there that demonstrated Roche’s respect for Scancell ... look at this from 2016 ...

Of course these mechanisms are very complex and I've probably over simplified it to a certain extent but trying to just give a sense of what's going on for those new to immunotherapy ..

Pd1 / Pd-l1 terminology is a reference to sites on tissue/tumour cells (pd-l1) and on the immune system t-cell (pd-1).

Think of them as a lock and a key that when engaged cause the death of the tissue/tumour cell. A process happening naturally throughout the healthy human body all the time.

When a tissue cell 'goes wrong' it can switch off this process and evolve into a cancer/tumour cell ..

There are several checkpoints like this..

Any treatment that interferes with these 'pathways' are called checkpoint inhibitors.

Brilliant video .. there is an interesting comment in there very relevant to recent discussion here ..he mentions that recruiting 4% of the t-cell population in melanoma patients is a success ! I'm not sure on the numbers scancell recruit but looking at the scib1 results, I'd love to know !! Do you know if that's ever been disclosed !?

Yes, my goodness 10,000 posts ! As you can tell, the science really interests me.. and during those posts many thanks to all those who have contributed to our discussions and helped me with my understanding!!

The news Nom posted yesterday is all over the place today, and being reported slightly differently depending on which one you read ... but the numbers are the same in all .. I'd summarise .. promising but not a home run !! And also in one was the usual caveat ...

"Cost is another concern. Tecentriq is $12,500 a month. The chemo in this study was Celgene’s Abraxane, which costs about $3,000 per dose plus doctor fees for the IV treatments. "

Yes, interesting and does highlight that a few extra months is seen as a success but ... plenty of room for further improvement!

What’s interesting about the scib1 trial is that it observed an increase in pd-l1 on the tumour cells ... I.e. they try to ramp up their own protection as a result of an increase of t-cells trying to attack them !

It’s all very interesting stuff !!

Mechanism wise .. think of the tumour cells knowing their assailants and building a barrier against them (t-cells) as they try to attack ! Switching these defences off (using checkpoint inhibitors) allows the immune system to mount a more effective response.

However, the down side is they can cause the t-cells to attack completely healthy tissue hence the side effects ..

Checkpoint inhibitors are a huge advance in themselves regarding cancer treatment .. but they could be made better by immunobody (in my opinion) and in very late stage cancer done without by moditope !? (Again in my opinion)..

Just looking at those numbers and I’m not following them ..

I don’t know what proportion of these t-cells treatments need to recruit to make a difference ... but for the average person with 4.4 litres of blood (that’s 4,400,000 mm cubed) which in a healthy individual equates to up to 7,040,000,000 T cells ... and I believe when an individual if fighting an infection this number can go up!?

It’s easy to see why it’s not an easy process and if you get it wrong it can go very very wrong I.e. issues with early car-t therapies ...

But if you get it right, you have a very powerful treatment .. scib1 ! (In patients with immune capabilities)..

As for shouting from the rooftops ... I’m happy with quiet !

I think what's important to consider is just how many t-cells we are talking about ..

That's per cubic mm !!

The aim to command this army in a safe way is a huge undertaking .. or even build a new army capable of making a difference is an even bigger one !!

Manipulating individual cells is one thing ... orchestrating millions or billions is another, no matter what the minute mechanism .. the result however of patients living when they by all accounts shouldn't be , says it all !!

The more cancer specific targets you can 'program' the better and more wide ranging your treatment will be .. for instance scib2 has 16 targets!!

The trick is, is the 'cancer specific' bit ! And that's not the end of it .. the bonds created by these targets need to be of very high avidity (in other words, strong)..

When scientists crack this specific and strong targeting, my money is on the immune cells winning the battle against unwelcome tumour cells .. (plus other stuff outside oncology) ..

It is a fascinating observation lindy happened upon indeed.

The focus of the market is directed to these cell therapies TCR, Car-t etc. however, these will be end of line last ditch therapies when patients immune systems have become utterly compromised, hence introducing new cells with specific duties ! (quickly!) (part of their issues currently is time to manufacture)..

This is (cell therapy) arena is a lucrative market. However, scancell are involved in so much more than this end of line treatment.. with harmless immunobody and core moditope technologies looking good in patients who still are able to mount immune responses, its amazing you can buy into all this for less than 10p !!
