Realism about Dx on a thread about a hypothetical change in the trial design doesn't add to the Tx conversation taking place.

If you want realism about Dx then the best bet is forget about it - we'll sell some stuff through our acquisitions and gain some useful cash flow and that's about it. Affimers haven't been meaningfully commercialised since the inception of the business. Coris enables that, but it'll be years away if we're lucky. Covid rearing its head will just mean we'll gain some additional revenue (but pay 50% of that to the previous owners).

Bein, happy doing their jobs for them - I think it's a great idea to miss out 1b.

Thanks RD, can you let Alan know? The original logic behind splitting phase 1 isn’t clear to me as 1b is looking very similar - and in that case, why not go straight to 2 and expedite the trial and time to approval.

On that basis I’d redesign, add a few and rename it to phase 2…

Thanks JT. Is there a difference between 1a looking like 1b and 1b looking like a 2? Does the name matter? Basically we’re talking 1 further stage, not 2. I agree 1b in the design we’ve seen is looking like a 2 already. What box needs ticking to move to phase 2? I think there’s a good chance of approval on that basis as you’ve said, given what we’ve seen of the data so far. To quote Alan, it would be ‘transformative’ if we were able to commercialise AVA6k in any accelerated way.

If they’re recruiting STS patients now that the USA and Tap are onboard, hasn’t 1a morphed in to 1b? Assuming a few more DEs then they could be plenty data. So, no need to split out phase 1 and we can crack on with phase 2 ‘shortly’?

They’re busy deciding just how explosive missus miggins diarrhoea needs to be to classify as a DLT. If she spray paints her living room to match her bathroom that’ll be the MTD reached.

Fiona has said they’re measuring dox and they’ll no doubt know it’s going to be this or the next cohort. Although.. If they decide to go further we’re reaching insane amounts of dox that can be safely administered. Some time in the next few weeks we’ll know if we’re off to 1b or another few months of the same conversations…. Might even see some data soon and have something to properly discuss.

a wise person said follow the money. by a wise person, i mean a conspi****** lunatic, but the point stands. dox is cheap, it’s a standard of care. anything personalised will be prohibitively expensive for a long while. in the intervening period, something like ava6k can do very well.
