Oversimplified the interim analysis could show 3 things.
1. Indication of Zero or negative effect at interim stage.
Trial stopped on grounds of futility. No realistic chance of proving benefit.
2. Indications of benefit at interim stage, not statistically significant, but with a reasonable chance of showing statistically significant benefit by completion of trial. Trial continues, to see if statistically significant benefit becomes apparent.
3. Treatment knocks it out of the park by interim stage, showing statistically significant benefit even with the lower sample size at interim stage. Trial stopped, placebo patients get the treatment. Happy days.
Passing "futility" just means you got past stage 1. It doesn't mean you have shown statistically significant benefit. It just means you've shown that it's still possible you will show statistically significant benefit by completion of the Trial.
