Current news about BQ.1.1 and the other scary variants is a perfect case in point and has been highlighted by other posters. In the US it doubled to over 10% of infections in the week ending October 15th. Fauci has started to sound the alarm that if it does take hold it will wipe out the remaining Mabs - but is it being spoken about ? Did his warning make any of the front pages over there or here. Sleep walking to disaster is overly dramatic but if the doubling continues and is as bad everywhere then we really are in trouble again.

I've made my point on the LC trial (or possibly even an EUA !!!) a little earlier. My post wasn't negative but it was much more likely than your Cinderella fantasy. I'm telling you it's a red herring and that there's still a great big fat Moby Dick out there called Covid.
I'm also not interested by the Jannsen colaboration unless it suggests Jannsen came to Southampton to see Synairgen on other business and I also posted that Poly chased a huge amount of money with a much smaller amount of money to both lower their average, gain a huge amount of leverage and support the sp during this period of uncertainty. Imagine where the sp would be without those buys...

We need to get in with Remdesevir. It is EUA'd for hospitalised patients well before Dexa gets a go. This is where we need to be, and I've been saying it for a long while. Like Ghia says they could even have raised funds for this specific trial right after the Sprinter deep dive. We haven#t lost faith in the drug but we're stuck with a pragmatic BoD sitting by the phone wondering if anyone important read Mani's most recent post. We 're not gonna be the mystery LC drug unveiled by David Strain but we don't need to be - the deep dive gave us a treasure map with a clearly marked X and since then we've been standing outside Homebase wandering whether they sell shovels.

NIHR " The Panel recommends using remdesivir ( +SNG001) for the treatment of COVID-19 in patients who do not require supplemental oxygen and who are at high risk of progressing to severe disease ().
Remdesivir (+ SNG001) should be administered for 5 days or until hospital discharge, whichever comes first.

I'll eat my head if we are included on Recover or anything else for a straight-up LC trial. And I'll livestream it. Why would anyone support an LC trial when the trialist is also a prime Covid-in-hospitals candidate for exactly the same cohort of patients. You'd be losing potentially 30% of your best trialists because they're dead or too severaly affected before you start giving them SNG.

It was SNG that was in short supply - there was no comparator for P2, but a long time ago as you say SNG and I can't be arsed to trawl back for it. We have to hope the NIH's delay with results is meaningful.
I don't want to be perceived as a monomaniac over this but Covid is still here - we do still have the bst hospital data and the only LC data - so let's get back onto a fuc king hospital trial asap and get this candle lit !!!

The limited roll out of Activ 2 sites has been posted on here many times Sparky. I can't remember what the original source was but it is undeniably the truth.
My point Tommy D about this limited roll out is that if we'd recruited at the speed of SAB - with our headstart - we could have been through P3 before Omicron arrived. Sab's ttial was paused when they had 80% of the P3 patients recruited - and of of course it then failed against Omicron - but SNG could have passed P3 and as we know it works against everything including Omicron . Dubious still whether NIH would have gone ahead with an EUA in te new climate - but the data would have been there and collected from Delta patients.
And remember i only posted this to contest Tommy's assertion that NIH was to blame for the delays.

But let's start by Not scapegoating the company's greatest ally since this whole business began.

Tommy's last post literally discussed it. And you banging on about LC as a next step when you are unable to answer the questions set about what kind of trial. If anyone has a narrative it's the two of you guiding us away from the mnain event and in the process excusing the company in advance for not managing to find a funded P3 despite all the good results it possesses.

NIH offered potentially 100s of sites to Synairgen to begin Activ 2 -P2 and the company couldn't take them up because they hadn't produced the drug in sufficient quantities. As I posted before - if SNG was trialed at the same speed as SAB-185 we'd have not just been through P2 but also finished P3 before Omicron ever crawled out of that ditch in Africa.
And we'd have been authorised long before Omicron started to dent other therapeutics. Same can be said of Sprinter. Months waiting - unnecessary reduction in trial size and power and again we could have been EUAd in the hospital setting long ago.
The delays are entirely the fault of the company - which now finsllay finds itself with 10s of 1000s of treatments but no trial.
Your narrative that Covid is not the only game in town may reflect the company's new approach but if they give up on Covid - or are unable to find the next Covid trial just as another very difficult and deadly winter begins it will be the greatest failure.

Well put Brysoa - I can't see SNG on a specific LC trial for these very reasons. SNG could help to clear residual Covid from the lungs - which is an important part of the first stage LC - but nothing to suggest it will help clear it from other organs or help with any of the major repair work in affected organs.
Or LC data strongly supports a further large platform trial for Covid and if successful SNG would both saving lives, freeing up beds and as a biproduct reducing LC in the next wave of severely ill patients.
So you ask if EUA is possible just because of the LC reduction data and in answer to this you'd probably say no. Whilst still a huge ongoing issue LC is nowhere near as prevalent in Omicron patients - 4.5% compared to Delta 10.8.

It was a good thought Paladin - and as you say we do hope to be eventually helping exactly the same people - and others either before admission or after they arrive in hospital. In an ideal world the immuno-compromised would receive Evusheld as a propylactic and then find SNG waiting for them if things still go wrong.
It's good to see a correctly sized and specifically targeted trial producing a statistically significant result. And hopefully our next big trial will do the same. Not sure how many immuno-compromised patients have died of covid but it must be awful to know that you WILL do badly if you catch it but can't receive the one therapeautic successfully trialed for your group.
Fruiits - yes the penny-pinching remark may have been just my prejudice - but the reasons they give are pretty flimsy.

The FDA made its first purchase in December 21 and another 500k doses in Jan 22. For the MHRA to suggest " it is highly unlikely that supply could be made available to the UK for all these patients in autumn or winter 2022 " is about as lame as you can get. Sorriot said as early as May that the company was waiting and ready to supply to its home country.
"Insufficient data" is also a pretty pathetic line when what they mean is they don't have the dat fro how it will perform if new variants arrive. And of course that data is unknowable

Evusheld was trialed against only unvaccinated patients - which supports its use for immuno-compromised patients. It trialed 910 patients, of whom 90% were at high risk of progression to severe COVID-19 due to co-morbidities or age
"Severe COVID-19 or death occurred in 18 (4%) of 407 participants in the tixagevimab–cilgavimab group versus 37 (9%) of 415 participants in the placebo group (relative risk reduction 50·5% [95% CI 14·6–71·3]; p=0·0096). "

SNG has no statistically significant data that would assist these groups as far as I'm aware - except the small signals from our own home trial which took place almost entirely before the vaccination programme, and which were assisted by the A2 data but not to a level that would support an EUA application.

The UK govt approved Evusheld and then did something extraordinary in refusing to purchase or supply it due to concerns about how long it might work against future variants. 32 countries - including the US - took the contrary view and have been injecting immuno compromised ever since. None of the therapeutics designed for Covid would ever have been administered if the same approach was adopted that says " you can't have it because we don't know what's coming down the road ". We approved Sotrovimab and rolled it out ( as one example) and have freezer-loads of the stuff remaindered as it was wiped out by Omicron just a few months later.

Evusheld's trialing story is an interesting parallel though. The P3 launched at the same time as Sprinter./ Imn different settings of course but 910 patients versus 623 - with more sites but fewer countries. Last patient dosed 6 months after commencement versus and EUAd in the States before the end of 2021. Less than a year end to end. And just a month after Synairgen announced LPD for Sprinter.

All of which is not to be critical in any way of the positive notes we've seen recently for SNG - or your continuing trememndous efforts Paladin - but to suggest that comparing apples with pears may not be helpful
