I've been a manager on building sites since the 80's and I've never had any issues on my sites. I could "smell" bricklayers a mile off within 5 minutes of meeting them. The bricklayers I left to their business the rest I kept a close eye on or got rid of, I stuck by the men that produced good work.
Panels that were substandard I kicked down literally.
I had no problems with quality of mortar, I tested, tested, tested and had the ready mix company test as well.
There no excuse from the management side apart from inexperience or cost cutting from accountants(sorry Joeking my lads an accountant and investor here)
My rant over :))
