this may not be a popular message on this board but remember i have probably got a higher shareholding than many here... as this has played out, i feel i have got more clued up on the therapies and timing of when they are administered. So Regeneron is very early before cell entry. Remdesivir lowers replication rate once in cells. Dexa is when virus is serious and starts to attack other organs so steroid to try protect those organs. However, the majority of the public are told to self isolate at home. By the time they get to hospital, and are admitted everyone is serious. So where does Syn fit in? If you are on Dexa, then it would be contra indicated to take it as Dexa is dampening the immune system so why would you want to increase it with interferon beta. Thus, in current situation with dubious testing, in UK by time you get to hospital you cant really have Syn as you are on Dexa. You are very ill. Thus Syn needs to be pre hospital unless the rules of admission change. I think it can be given with Remdesivir but more to the point, Syn in current set up needs to be pre-hospital admission. Thus the in-home data is critical. Further, we need to know if its ok to give it to people - presumably it cannot be given to everyone (as they may be heading to hospital quickly and be counter indicated). On the COPD stuff, this was OK, as you were either in hospital or able to see GP without infecting them with a deadly virus. But now you are infected with a virus. So you are told to not go to the gp and to stay at home until you are very sick. This is increasingly tricky as at some point it will start to harm you and may be counter indicated. Thus seems to me, that SYn need to help people understand how it can be given safely early at home unless they set up hospital clinics for people with early symptoms. Really the nightingale hospitals need to be converted into early symptom places, so that you can be checked and popped onto Syn as otherwise i can see this might become tricky. Instead of Boris saying stay inside 14 days if you have symptoms, we'd be better off saying get to hospital as soon as you have symptons so we can see if you will benefit from Syn. WE need to get to people earlier before admission as on admission, you are very sick and too late for Syn. The structure of the thing is wrong - to protect NHS we need thousands turning up for minor tests to then get Syn or others. This will stop people being admitted. Thus the in home test is fundamental. Cant see it being used in hospital under current structure as everyone too sick.
