The most interesting rad across relates to dosage - which remains the unknown for the SNG trials. Surfactant started at a disdvantage to SNG as it has never been administered by nebuliser for Covid patients, so they had to assess and adjust as they went along. 4 cohorts on difft doses - dose reduced by 50% after the first 2 cohorts and time extended from 24hr to 48/72hr. Mixing and matching to find a sweet spot which ultimately eluded them.

We can assume Synairgen will also play mix and match with SNG's dosage and course length. SNG001's 14 day Sprinter delivery is obviously not going to be appropriate and could be reduced to something approaching this 48/72 hour window. You'd also imagine they will want to experiment with higher dosages.

The original questions remain. Not in a negative way but posting about another drug using the Aerogen is hardly of relevance. What matters is dosage - bearing in mind what TP has just written about the scary advice on toxicity - and whether the regulators have what they need to pass it on the nod

Aerogen are the global leaders in supplying drugs to ventilated patients - via the Aerogen solo - so yes Fruits it HAS been done.

Or is it actually much easier - as the patient population is pre-selected. None of the fine tuning they're engaged in with Casanova or all the other metrics. If the regs say yes - everyone on a ventilator gets a puff

I'm also interested in this as per my previous posts on the matter. Agree it's potentially much more complicated than designing mini trials in the other areas of interest. With the other trials you're looking at various known factors from Copd, Sprinter and hopefully UNIVERSAL - and you're giving them the standard dosage from Sprinter and using the Aerogen Ultra.

To reach ventilated patients - without any previous trialing history in this cohort, regulators have to be convinced of the potential efficacy, safety and even dosage. It's possible some of the UNIVERSAL trialists will end up on ventilators and provide useful data which could be supportive.

The Aerogen was chosen to replace the I=neb before Sprinter partly because it had the ultimate potential to be used in ICU and with ventilators. In Solo mode it just plugs in to the tubes - altho there was a blanket ban on aerosol triallaing in ICU during the pandemic and of course Synairgen still thought they had a general population blockbuster on their hands when they designed Sprinter and had no need to consider ventilation .

But how do they convince the regulators to let them do it ? And do they just go with standard dosage for 14 day course ? I don't know enough about the process to hazard an opinion and would welcome anyone with relevant knowledge, who can explain how the company will be addressing this. Would the Sprinter deep dive be enough on its own to allow a regulator to say - in this very poorly population - "ok - you can try it "
