The shared care records are just that. they are individual records that can be accessed by health, social services and community services. They are , if they exist a blunt record of a parsons personal information, medication, GP, etc
We need each person in Uk to have a shared record so that each record can then be enriched with information on their specific journey, pre diagnosis , diagnosis, and treatment of specific symptoms that can be shared with everyone involved in their care, and future care.
Its FDBK and others who are evolving the pathways that will be included in patients shared care record.
As we know at present they are developing the first " breathlessness" pathway, and if successful could be rolled out across the NHS shared care records, of those who have symptoms of breathlessness, for whatever reason.
the shared care records are the basic item, to enrich the records patients will have pathways to care, that can be shared.
