ADCs are trying to solve the same problem that pre|CISION is trying to solve – that of targeting a therapy to the tumour and avoiding systemic toxicities. pre|CISION addresses some challenges that ADCs face, namely: ADCs need to target an internalising tumour biomarker, expressed at very high levels on the surface of the tumour cell, and this affects the amount of drug that can be got into the cancer cells to kill them whereas pre|CISION therapies do not need to be internalised to release the drug. By targeting release of the payload in the tumour microenvironment, there will be a bystander effect i.e. all cells within the vicinity of the payload will be killed. ADCs are, in general, complex and expensive to manufacture, whereas pre|CISION chemotherapies are relatively simple and affordable. There is therefore scope for both approaches to coexist.
