Tofacitinib is a mainly Jak3 inhibitor. jak 3 is known to have serious side effects and has a black box warning issued by the NDA. Furthermore Jak 3 it does not inhibit Interleukin 10 and more importantly Interleukin 6. To a lesser extent Jak 1 and Jak 2 provide a limited control of Interferon gamma and Interleukin 6. The compound is far from being selective enough. It does not press all the keys that are required and presses some of the keys that are not required. The words chocolate and fireguard come to mind.
The problems are there are too many Bengal Lancers, ie pharmaceutical companies who are trying to get their compound into the market for big bucks of a worldwide treatment. They dont give a monkeys how many people succumb to serious side affects as governments have authorised their use. of course no suggestion of back handers to decision makers.
Tofacitinib is required by the FDA to have a boxed warning on its label about possible injury and death due to problems, such as infections, lymphoma, and other malignancies, which can arise from use of this drug. Serious infections leading to hospitalization or death, including tuberculosis and bacterial, invasive fungal, viral, and other opportunistic infections, have occurred in patients receiving Tofacitinib. Epstein Barr virus-associated post-transplant lymphoproliferative disorder has been observed at an increased rate in renal transplant patients treated with tofacitinib while on immunosuppressive medications. Patients are warned to avoid use of tofacitinib citrate during an "active serious infection, including localized infections." Doctors are advised to use it with caution in patients who may be at increased risk of gastrointestinal perforations. Laboratory monitoring is recommended due to potential changes in lymphocytes, neutrophils, haemoglobin, liver enzymes, and lipids.
Epstein Barr virus is cosidered to be the cause of around 200,000 world wide deaths of cancer each year.
