If it was solely down to the SDMC this would have been done by now, going by the way previous announcements have been structured. Surely this delay can only be due to changes to the trial protocol having to be approved by the Ethics Committee, who will want, indeed absolutely need, to review the trial data so far in order to approve an increased maximum lifetime cumulative standard doxorubicin plus AVA6000 exposure and possibly a different dose escalation regime.

Such changes would be needed for Phase 1b dosing as well if the dose chosen is to be higher than 200mg/m². A way of looking at this is that any delay between Phase 1a and Phase 1b due to protocol change has been brought forward.

DE1 decision was announced by RNS on 3 February, probably too soon before the Vox interview to conclude statistically significant inter-dose consistency. Moot point as to whether there were too few in cohort 1 to conclude statistically significant intra-dose consistency there, the decision to escalate being taken, as it must be, based on analysis of the data from cohort 1...

The trial started in August 2021.

Even if the SDMC stick with a 40 increment to 240mg/m², that will be 240x54/80 which is 162mg/m² and only 2 (max) cycles would be allowed in order to stay under the maximum cumulative lifetime exposure if that is 450mg/m². Three cycles would be 486mg/m².

If they go for a higher increment then definitely only two cycles would be possible.

So... maybe the delay between the RNS and announcing the next DE could be because they want to up the maximum cumulative lifetime exposure and that would require a change to the trial protocol and that would have to be approved by the Ethics Committee and hence the lapse between deciding to do it and being able to do it So, watch the ClinicalTrials site for updates.

But the FDA were considering the efficacy-side effects balance.

I did wonder why no new DE details with the RNS. Assumed it was because the SDMC had been considering the data so far (which Avacta had surely put together in presentation form) and by the time they'd done that and returned from the pub it was time to go home, so decision to be taken at the next meeting.

There is an alternative - Avacta becomes a subunit of a pharma major and Alan stays in charge and directs development.
