Today, we would like to share the findings of a study evaluating our 𝗥𝗘𝗦𝗜𝗦𝗧 𝗔𝗖𝗜𝗡𝗘𝗧𝗢 test, designed to detect key carbapenemases - OXA-23, OXA-40/58, and NDM - in 𝘈𝘤𝘪𝘯𝘦𝘵𝘰𝘣𝘢𝘤𝘵𝘦𝘳 species, addressing the critical challenge of carbapenem-resistant 𝘈𝘤𝘪𝘯𝘦𝘵𝘰𝘣𝘢𝘤𝘵𝘦𝘳 𝘣𝘢𝘶𝘮𝘢𝘯𝘯𝘪𝘪 (CRAB). CRAB is an extensively durg-resistant pathogen posing a significant global threat, leading to a high mortality rate associated with antimicrobial resistance.

📍The RESIST ACINETO test was evaluated according to our recommendations for its ability to detect carbapenemases directly from both bacterial cultures and positive blood cultures.

📍The research involved 121 clinical isolates of 𝘈𝘤𝘪𝘯𝘦𝘵𝘰𝘣𝘢𝘤𝘵𝘦𝘳 spp., including 104 carbapenemase producers, selected from the French National Reference Center for Antibiotic Resistance and the University Hospital of Reunion Island. The isolates were identified to the species level using MALDI-TOF or by sequencing.

📍For the 97 carbapenemase-producing strains targeted by the test, all enzymes were correctly detected (27 OXA-23 type, 14 OXA-24/40 type, 6 OXA-58 type, 21 NDM-type, 16 OXA-23+NDM-1, 3 OXA-58+NDM-1 and 10 others carbapenemase combinations).

📍 7 NDM-producing strains were considered positive.

📍No false positives were observed for the strains producing a non-targeted carbapenemase or other beta-lactamases.

📍The test demonstrated 100% sensitivity and specificity when used on bacterial cultures, and 99% sensitivity and 100% specificity on blood cultures.

✅ The ability to rapidly and accurately detect carbapenemase-producing 𝘈𝘤𝘪𝘯𝘦𝘵𝘰𝘣𝘢𝘤𝘵𝘦𝘳 spp. is crucial for optimizing patient treatment and implementing effective infection control measures, especially for countries with a high incidence of CR-𝘈𝘤𝘪𝘯𝘦𝘵𝘰𝘣𝘢𝘤𝘵𝘦𝘳 species isolates.

The RESIST ACINETO test's performance supports its integration into diagnostic workflows, potentially improving outcomes for patients with bloodstream infections caused by these multidrug-resistant pathogens.

In routine laboratory settings, various diagnostic methods are employed in combination to identify pathogens and determine their susceptibility to antibiotics, guiding clinicians in selecting the most appropriate treatments.

💡 Antimicrobial Susceptibility Testing (AST) methods like disk diffusion tests and broth microdilution assess how sensitive bacteria are to different antibiotics. The results form an antibiogram - a profile that helps clinicians select effective treatments. However, these tests can take 24 to 48 hours, potentially delaying optimal therapy.

💡MALDI-TOF mass spectrometry has revolutionized microbial identification by analyzing unique protein fingerprints of bacteria and fungi. It provides rapid and accurate results, significantly reducing identification time. Yet, it doesn't indicate antibiotic susceptibility, so AST remains necessary.

💡Polymerase Chain Reaction amplifies specific DNA sequences, enabling rapid detection of pathogens and resistance genes directly from patient samples. It delivers results much faster than culture-based methods, allowing timely initiation of appropriate therapy. However, PCR requires specialized equipment, is expensive, and may not detect all resistance mechanisms.

➡️ These methods are used together for maximum accuracy: specimens are cultured, identified with MALDI-TOF, and tested for antibiotic susceptibility via AST. Despite this integrated approach, the process can still take days, delaying effective treatment and potentially contributing to the spread of resistant bacteria.

📌 The Role of Rapid Tests

To address the need for even faster diagnostics, rapid tests can easily be incorporated into laboratory routines. hey significantly reduce the time to results, allowing clinicians to initiate appropriate antibiotic therapy sooner. This prompt intervention can improve patient outcomes, reduce the misuse of broad-spectrum antibiotics, and decrease healthcare costs associated with prolonged hospital stays and ineffective treatments.

By enhancing the speed and accuracy of diagnostics through methods like AST, MALDI-TOF, and rapid tests, laboratories can provide timely and precise information to healthcare providers. This integrated approach is crucial in the fight against AMR, ensuring that patients receive the most effective treatments promptly while preserving the efficacy of existing antibiotics.

📌 Our 𝗢.𝗞.𝗡.𝗩.𝗜. 𝗥𝗘𝗦𝗜𝗦𝗧-𝟱 test is a breakthrough in rapid diagnostics, detecting five key carbapenemase enzymes: OXA-48, KPC, NDM, VIM, and IMP.

📌Our 𝗥𝗘𝗦𝗜𝗦𝗧 𝗔𝗖𝗜𝗡𝗘𝗧𝗢 test addresses the challenges posed by multidrug-resistant Acinetobacter baumannii. This rapid assay detects the presence of NDM, OXA-23, OXA-40, and OXA-58 carbapenemases, which are notorious for causing severe infections in healthcare settings.

📌Our 𝗥𝗘𝗦𝗜𝗦𝗧 𝗖𝗧𝗫-𝗠 test specifically targets the detection of the major groups of CTX-M-type extended-spectrum beta-lactamases (ESBLs), among the most prevalent enzymes conferring resistance to third-generation cephalosporins.

Reliable, rapid (results obtained in 15 minutes!), easy and ready to use, our rapid tests enable clinicians to quickly identify the major enzymes responsible for antimicrobial resistance in the most dangerous group of ESKAPEE pathogens. This ensures timely and appropriate therapy while preventing the spread of these pathogens.

📌Additionally, we have developed a range of culture reagents kits: 𝗥𝗲𝗦𝗖𝗮𝗽𝗲, designed to enrich carbapenemase-producing Gram-negative bacilli by rapid selective culture from rectal swab specimens, and 𝗥𝗘𝗦𝗜𝗦𝗧-𝗕𝗖, designed to directly process positive blood cultures, both products allowing to improve the detection of beta-lactamases on a bacterial extract.

This week is Antimicrobial Resistance (AMR) awareness week, highlighting the global health threat of microorganisms which have evolved to resist the effects of antimicrobial drugs, making infections harder to treat. The overuse and misuse of antibiotics is accelerating the rate of resistance occurring.

Carbapenemase-producing Organisms (CPO), and more specifically, Carbapenem resistant Enterobacteriaceae (CRE) represent a major public health concern worldwide due to their broad spectrum of resistance to antibiotics, leaving very few options for the management of infected patients.

As we continue to observe World Antimicrobial Resistance Awareness Week, it's essential to delve deeper into how bacteria🦠 develop resistance to antibiotics 💊, through four main processes:

𝟭. 𝗘𝗻𝘇𝘆𝗺𝗮𝘁𝗶𝗰 𝗱𝗲𝘀𝘁𝗿𝘂𝗰𝘁𝗶𝗼𝗻 𝗼𝗿 𝗺𝗼𝗱𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗼𝗳 𝘁𝗵𝗲 𝗮𝗻𝘁𝗶𝗯𝗶𝗼𝘁𝗶𝗰:

Some bacteria produce enzymes that inactivate antibiotics. For instance, beta-lactamases are enzymes that break down beta-lactam antibiotics like penicillins and cephalosporins, rendering them ineffective. By destroying or modifying the antibiotic molecule, bacteria can survive and continue to multiply even in the presence of these drugs.

𝟮. 𝗔𝗹𝘁𝗲𝗿𝗮𝘁𝗶𝗼𝗻 𝗼𝗳 𝘁𝗵𝗲 𝗮𝗻𝘁𝗶𝗯𝗶𝗼𝘁𝗶𝗰'𝘀 𝘁𝗮𝗿𝗴𝗲𝘁 𝘀𝗶𝘁𝗲:

Antibiotics work by binding to specific targets within bacterial cells. Bacteria can mutate or modify these target sites so that the antibiotic can no longer bind effectively. This means the drug can't interfere with cell wall synthesis, allowing the bacteria to grow unhindered.

𝟯. 𝗘𝗳𝗳𝗹𝘂𝘅 𝗽𝘂𝗺𝗽𝘀 𝘁𝗼 𝗲𝘅𝗽𝗲𝗹 𝘁𝗵𝗲 𝗮𝗻𝘁𝗶𝗯𝗶𝗼𝘁𝗶𝗰:

Bacteria can possess efflux pumps - proteins that actively transport antibiotics out of the cell. By pumping the antibiotic out, the intracellular concentration remains too low to be effective. Efflux pumps essentially help the bacteria "spit out" the antibiotic before it can do any damage.

𝟰. 𝗥𝗲𝗱𝘂𝗰𝗲𝗱 𝗽𝗲𝗿𝗺𝗲𝗮𝗯𝗶𝗹𝗶𝘁𝘆 𝗼𝗿 𝗽𝗿𝗲𝘃𝗲𝗻𝘁𝗶𝗻𝗴 𝗮𝗻𝘁𝗶𝗯𝗶𝗼𝘁𝗶𝗰 𝗲𝗻𝘁𝗿𝘆:

Some bacteria can alter their cell wall or membrane structures to prevent antibiotics from entering the cell. Changes in porin proteins, which form channels in the bacterial outer membrane, can decrease permeability. This limits the amount of antibiotic that can penetrate the bacterial cell, reducing its effectiveness.

👉 Understanding these mechanisms helps researchers and healthcare professionals develop new strategies to combat AMR, for instance in the development of new antibiotics that can bypass resistance mechanisms or in creating policies for appropriate antibiotic use.
