National clinical trial to treat hospitalised patients with COVID-19

A new clinical trial led by Queen’s University Belfast and the University of Warwick seeks to find alternatives to ventilators to treat patients who are critically ill with COVID-19.

Researchers will deliver a clinical trial that aims to recruit approximately 4,000 patients in order to find effective alternative solutions for patients with COVID-19 to reduce the need for treatment with a ventilator and improve patient outcomes.

A recent report estimated that 30% of Covid-19 hospitalised patients are likely to require mechanical ventilation.

The UK is facing a shortage of both equipment and trained staff to operate the ventilators and therefore it is crucial to find effective, alternative ways to treat patients.

Leading cell therapy clinical trials to help improve outcomes in COVID-19 patients

Researchers at Queen’s are leading a UK-wide clinical trial, offering an innovative cell therapy treatment for COVID-19 patients with acute respiratory failure.

This clinical trial, led by Professor Danny McAuley and Professor Cecilia O’Kane, will involve the use of MSCs, a type of cell derived from human tissue such as bone marrow or umbilical cord (which is otherwise discarded after the baby is born), to treat the injury to the lung caused by COVID 19. MSCs are a novel treatment that have been shown in experimental models to reduce inflammation, fight infection and improve the repair of injured tissue.

The trial has been identified by the National Institute for Health Research (NIHR) as a national urgent public health study. It is one of a number of COVID-19 studies that have been given urgent public health research status by the Chief Medical Officer/ Deputy Chief Medical Officer for England.

New research alliance with Fusion Antibodies to seek therapeutic and diagnostic approaches

The Northern Ireland Coronavirus Antibody Development Alliance (NICADA) aims to build upon Fusion Antibodies’ expertise in antibody and protein engineering to develop new therapeutic molecules that can be evaluated for their ability to neutralize SARS-CoV-2, the virus that causes COVID-19 disease.

Working with Professor Ultan Power in the Wellcome-Wolfson Institute of Experimental Medicine, Professor Chris Scott in The Patrick G Johnston Centre for Cancer Research and Dr Fuquan Lui in the School of Biological Sciences, at Queen’s University, Fusion Antibodies will prepare potential antibodies and engineer them specifically to neutralise the virus.

on the combination of nanoparticle-Actinomycin D (NP-ACT D) with anti-interleukin-6 receptor monoclonal antibody (anti-IL-6R) as a potential therapy for management of COVID-19 disease. The underlying invention concepts are based on the hypothesis that a combination of an antiviral drug controlling proliferation of COVID-19, with an anti-inflammatory agent (e.g., anti-IL-6R) suppressing a possible 'Cytokine Storm' may provide immediate relief to severe cases of COVID-19 patients.

Tiziana Life Sciences PLC's (LON:TILS) founder and chairman Gabriele Cerrone tells Proactive London's Andrew Scott they've filed a patent application around the combination treatment of Nanoparticle - Actinomycin D with its Anti-Interleukin-6 Receptor Monoclonal Antibody for the treatment of coronaviruses.

It follows last week's update that they've acquired the slow release, nano-particle formulation of Actinomycin D which is an antibiotic drug used to treat cancer.

The formulation was developed by Dr Kunwar Shailubhai, the company’s chief executive, during his time at Rasna Therapeutics.

Cerrone says the combination of the drug as an anti-viral along with their anti-inflammatory agent may present an immediate therapeutic option for COVID-19 patients.

TILS are very well funded with $10 million cash in the bank they raised last month and the £500k plus they raised last week on share warrants exercised, and have also have the provision of a further $20 million o, if and when needed. If Sanofi confirm a therapeutic benefit and response to the inhibition of IL-6 for severe lung conditions in Covid-19 then of course this will attract much interest from potential suitors to JV TZLS-501's development as well as charitable funding from US and or UK agencies desperate to find effective treatments for Covid-19......watch this space....news from Sanofi imminent and news will quickly follow from TILS, an absolute certainty imho....Gla holders, anyone betting short should be extremely worried!!! ;-)

Sanofi's CEO stated on Friday that patient trials news is imminent, and their General Meeting for shareholders is tomorrow and so news anytime now, and no doubt TILS will be eagerly awaiting Sanofi's update before releasing potential Transformational news on TZLS-501 . Tiziana, meanwhile, will start assessing the drug in patients as soon as is practicable and will administer TZLS-501 using its proprietary formulation technology.

It said the features of its drug candidate should provide it with distinct advantages other anti-IL-6R mAbs such as Actemra and Kevzara in treating severely affected COVID-19 patients.

The aforementioned advantages are TZLS-501s dual mechanism of action to inhibit signalling by the membrane-bound and soluble IL-6 receptors along with the rapid depletion of circulating IL-6 cytokine, a major cause of lung damage.

We are excited to move forward with our clinical development plan to expedite evaluation in patients as soon as possible", said Tiziana's chief executive Dr Kunwar Shailubhai.

Gabriele Cerrone, chairman, added that Tiziana only became aware of the potential of its anti-IL6R mAb after people in Naples with lung problems caused by coronavirus started to respond to treatment with Actemra.

We are sitting on one of best anti-IL6R monoclonal antibodies on the market but had no idea it could be used for Covid-19 complications on the respiratory system as no-one had ever tried it before.

I think it's pretty obvious if Sanofi confirm that IL-6 inhibitors can relieve the severe symptoms seen in Covid-19 patients, that untold resources will be thrown at every IL-6 inhibitor on the market, and it just so happens that TILS IL-6 inhibitor is one of the best ones on the market. Gl :-)

Anti IL-6rWe are developing a fully human monoclonal antibody (mAb) targeting the receptor for IL-6. This asset was in-licensed from Novimmune in January 2017.IL-6r mAb has a novel mechanism of action, binding to both the membrane-bound and soluble forms of the IL-6r, blocking the IL-6/IL-6r complex as well as depleting circulating levels of IL-6 in the blood.Excessive production of IL-6 is regarded as a key driver of chronic inflammation, associated with autoimmune diseases such as multiple myeloma, oncology indications, severe acute respiratory distress syndrome (ARDS), lupus, autoimmune inflammatory diseases and rheumatoid arthritis. Regeneron and Roche's rheumatoid arthritis drugs

The approach of using certain medicines already approved for rheumatoid arthritis to treat the lung inflammation in patients with severe Covid-19 "makes sense,"  Ho said. 

"The IL6-receptor antibodies," Ho said, "block one limb of the inflammation pathway… the question is, are you intervening soon enough?"

Regeneron and partner Sanofi, and separately Roche, are testing their drugs Kevzara and Actemra in hospitalized patients with Covid-19, after some promising data from a clinical trial of Roche's drug in China.

The problem, Ho noted, is the trial in China didn't have a placebo control.

"I think the mistake is not doing controlled trials," he said. "That's the only way you're going to be able to tell [if a drug is working], given most patients do recover."

Initial results from Regeneron and Sanofi's U.S. trial of Kevzara, which includes approximately 1,000 patients, are imminent, Sanofi spokeswoman Ashleigh Koss said Friday. Regeneron spokeswoman Hala Mirza noted about 450 patients are included in the phase 2 portion of the trial, which is what will be reported soon.

Roche, which started a trial in early April with plans to enroll 330 patients, expects results in early summer, said Shirley Dang, a spokeswoman for Roche's genentech unit.

"If there is a dramatic effect, it should be evident very, very rapidly given the number of subjects" in the trials, Ho said. "I suspect we're going to hear about all of them fairly soon."

Dive Brief:Sanofi anticipates study results for an arthritis drug repurposed to treat COVID-19 will be available "imminently," said Paul Hudson, CEO of the French drugmaker, which along with Regeneron is testing the immune-blocking therapy Kevzara in two clinical trials. The companies hope Kevzara could play a role in easing the severe respiratory symptoms some patients with coronavirus disease experience. But Hudson cautioned against expecting too much, describing it on a Friday conference call as a "fingers-crossed situation." Still, Hudson said Sanofi has taken its Kevzara drug production "to the maximum" in case the trials show the drug benefits COVID-19 patients. Fellow large pharma Roche, which is testing in COVID-19 a drug that works the same way as Kevzara, is taking a similar approach
