You do realise that even medical devices need clinical trials that test safety and effectiveness right? Enough data on safety now but effectiveness requires further trials. So a fundraising is coming.
So options:
1. Keep the product and spend a fortune marketing it, especially given it is not yet clinically proven that it does anything.
2. Sell / licence the product to someone who is prepared to do option 1. Price will reflect uncertainty and need for further trials.
3. Conduct further trials with an inert placebo to confirm that product does actually do something.
Options 1 and 3 will require a further placing. Option 2 will leave less value on the table, which I remind you is a exactly why the phase 3 study was done in the first place.

The more I think about it the more I worry. Time to exit tommorrow. Small companies like this normally break the pattern when something has really gone wrong. Profit warning in the offing?

Indeed, a reason to sell in itself.
Maybe the delay is because they are working through bad news, you are worried and have a large position and are trying to delay others with doubts from exiting while you do. Who knows.
Saying trading is in line with expectations is a perfectly normal and regular trading update. Macro events can't possibly be a factor for delay. Other companies are updating all over the shop. Smells fishy to me.
