If an investigational agent shows promise by demonstrating safety and reducing Covd-19 symptoms through 28 days following administration, the ACTIV-2 trial is designed to expand seamlessly from a phase 2 to a Phase 3 study to gather additional critical data from a larger pool of volunteers without delay. Phase 2 studies in ACTIV-2 enroll up to 220 volunteers, while exact enrollment size of Phase 3 studies will vary depending on mode of administration of the investigational agent. The adaptive nature of the ACTIV-2 trial allows for comparison of multiple interventions with a shared group of placebo recipients. In addition to assessing safety and effect on Covid-19 symptoms, ACTIV-2 studies also assess whether an investigational agent can reduce the amount of SARS-CoV-2 virus detectable in the nasopharynx.

While the study of SNG001 enrolled its first volunteer last week, the other agents should begin enrollments soon. Any that show promise through sufficient safety and reduction of COVID-19 symptoms over the course of 28 days following administration will be moved to a phase three study to gather data from a larger pool of volunteers immediately. Up to 220 volunteers can be enrolled in the phase two studies, but enrollment size for phase three studies can vary.

The therapeutics will be compared against a shared group of placebo recipients. Participants must have tested positive for SARS-CoV-2 — the virus that causes COVID-19 — within 10 days and started experiencing symptoms within eight days of enrollment. Beyond safety and efficacy, a side goal will be to determine if any of these agents can reduce the amount of SARS-CoV-2 detectable in the nasopharynx.

And two tiny biotechs, Germany’s Abivax and the UK’s Synairgen, have come a long way with their respective projects. Results due soon from both should show whether they can make a splash; the former's ABX464 is said to have both antiviral and anti-inflammatory properties, while the latter's SNG001 is an inhaled formulation of interferon beta-1a.

The strain - dubbed B.1.525 - has the E484K mutation that makes the current crop of jabs less effective. It also has the Q677H mutation on its spike protein - used to invade cells - prompting warnings from scientists this could give it further resistance to jabs. There are 33 cases in Britain so far and it was first identified in the UK, but scientists said this doesn't mean it evolved here because British labs do far more sequencing than other countries. The variant has been linked to travel to Nigeria, where 12 out of 51 virus samples analysed - or 24 per cent - were the new variant. For comparison, the UK has only found it in 33 out of 70,000 genomes sequenced, or less than 0.4 per cent.
