The ACTIV-2 master protocol(link is external) is designed as a Phase 2 trial that can expand seamlessly to Phase 3. The trial will enroll adults with COVID-19 who are not hospitalized and aims to evaluate safety, to understand if the investigational treatment can reduce the duration of symptoms, and to test if the treatment can increase the proportion of participants with undetectable virus. It will perform testing using nasopharyngeal swabs at specific time points. Multiple therapies, beginning with a monoclonal antibody but including other types of therapeutics, will be tested in ACTIV-2.

Petrofac secures Iraq contract extension

Petrofac’s Engineering & Production Services division (‘EPS’) has secured a one-year contract extension worth around US$80 million with a key client in Iraq.

The award is recognition of Petrofac’s successful eight-year track record of safe delivery as the incumbent operations and maintenance service provider. The facility which Petrofac will continue to manage, is one of the largest in the Gulf and handles around 55% of Iraq’s crude oil exports.

Topol: A paper just came out in Nature Reviews Immunology that reviewed four small ongoing trials and that multicenter study from China. All of them look quite promising. If you were to predict the future, do you envision that we all could have an inhaler with an IFN that we would take at first diagnosis or exposure? Do you think that's a possibility?
Iwasaki: As long as we can give the right dose without toxicity, that may be the future, especially in a preventive setting. For example, if your family member was diagnosed with a viral infection, you could potentially treat the rest of the family members with prophylactic IFN, and healthcare workers or people who are exposed to high-dose virus on a daily basis. That's what happened in China; they gave an inhaled IFN to healthcare workers and none of them were infected. So this may be a good thing to do in the future

2.2 Rationale
COVID-19 represents one of the most significant infectious threats to global public health security in over a century. In the absence of a licensed therapy for ambulatory patients with COVID-19 there is a need to assess new treatments which will prevent and effectively treat upper and severe lower respiratory tract (LRT) illness caused by the SARS-CoV-2.
Interferon-beta (IFN-ß) is a naturally occurring protein which orchestrates the body’s antiviral responses. Its role has been thoroughly elucidated in innate and adaptive immunity against viral infection. IFN-ß binds to and activates IFN receptors on the surface of cells, triggering the expression of interferon stimulated genes (ISGs) which will orchestrate and augment the host anti-viral response in the lung [1].
IFN-ß driven anti-viral responses have been shown to be compromised/deficient in older people and those with chronic airways diseases [2-4]. These and other patient groups are at higher risk of developing severe LRT illness which can be fatal and are major risk factors for death in COVID-19. The IFN-ß deficiency can be overcome

Phase II Period of Evaluation
In phase II, an investigational agent will be evaluated for safety, as well as for activity in reducing the duration of COVID-19 symptoms over 28 days, and SARS-CoV-2 RNA below lower limit of quantification in NP swabs as compared to control.

First, perhaps I may apologise to the noble Lord, Lord Bethell. On Thursday 4 February, I asked about antiviral drugs: Synairgen’s compound SNG001, an inhaled interferon beta drug, and the controlled trial which had been published in the Lancet and which showed considerable success. I also mentioned ACTIV-2, research established by the National Institutes of Health in the US. With the limits on time to reply to the Statement, I muddled Synairgen’s seemingly effective trial with ACTIV-2, giving the impression that it was a different drug. This made it impossible for the noble Lord to answer my question effectively and I apologise for that.

As many noble Lords have repeatedly affirmed from across the House, we must work together, particularly at times of national emergency. While I speak from these Benches, it is good to celebrate the spirit of proper collaboration of which your Lordships’ House is proud. This is a time of global emergency. Of course, we want to be vaccinated as soon as possible. With vaccines still not plentiful, it is natural to be anxious about ourselves and our families, but we must recognise those elsewhere globally, and particularly in poor nations. It makes economic sense, just as it does with climate change. It may be costly, but it is morally right, and the global fight is essential, not least because of our self-interest.
