I'm confused about cohorts 1 to 4 in the dose expansion. It seems like they have already decided the indications for the Q3W and Q2W dosings in Phase 1b. They may have a reason for this decision already having been made but, if they have, they haven't made that reasoning public, or at least I've not heard it.

That's the dose expansion. That's what threw me whenI first saw the plan. Dose escalation is anything from a nominal 42 to 84.

There is a commentary but I'm not sure I understand it. 3-6 patients per cohort across all four indications - 4 into 3 doesn't go so they must surely have some weighting for each cohort...

My understanding, from a slide I saw somewhere iirc, was that in the initial cohorts at least, where the emphasis is on proving safety and tolerability, the patient's indication will be selected by rota. I should think that would change as they start to look at efficacy more but in the early cohorts one indication is as good as any other. Well that was my understanding but, tbh, the plan was presented in a confusing way. Maybe there's a verbal description to go with the slide, but I've not heard one.

By 'the beast' I presume you mean AVA6103 for which there have been no cohorts as yet. I don't think the eligibility criteria have been announced yet—will be by RNS and in the yet to be created clinicaltrials.gov record—but as the first cohorts will be the first in human dosing, safety will take precedence so for ethical reasons I'm pretty sure they will take patients that are terminally ill, which is the normal criteria when the effect of the drug isn't known for sure.

Glad to see this series appearing. I found the discussion very interesting and informative. I did email CC last Wednesday requesting an explainer about how patient safety can be ensured at the start and during trials (starting dose level; identifyng contraindicatory—raised FAP—conditions; and, for faridox, monitoring cardiotoxicity/cardio-function now that the lifetime limit has been removed). Pretty sure this series was devised before my email, but hope that it will cover these aspects as they are of extreme importance to Avacta—you don't want patients dying from our drug on trial as that would be a nail in the platform's coffin, and no amount of spanner work would remedy that.

Good find. It's a seemingly intelligent (AI-generated?) summary of the importance of February's announcements. Where it fails however is in not looking beyond those announcements to other important information being provided by Avacta such as the collaboration with Tempus AI to identify susceptible indications, the ease of manufacture and cost advantages of the pre|CISION prodrugs, and the inhouse expertise in designing pre|CISION substrates to achieve desired pharmacokinetics. These very largely reduce the identified rsks. The only material risk that I can see are whether AVA6103 in humans will repeat the excellent characteristics of AVA6000, namely inert in the bloodstream, cleaved in the TME with minimal off-target toxicity, and inert/non-toxic byproducts - the capping group and self-immolative linker.
