These “core” protein biomarkers, most reliably measured in the cerebrospinal fluid (CSF), include (i) amyloid-beta peptide 1-42 (Aß1–42), reflective of cortical amyloid plaque formation; (ii) total tau, a marker of axonal degeneration; and (iii) phospho-tau (p-tau), representative of pathological tau hyperphosphorylation (4–7). While these CSF biomarkers have greatly advanced our detection of “hallmark” AD proteinopathy (4–7), they represent only a fraction of the intricate biology underlying the disease.
