Tim, both of the "players" in the no.2 category have a high unit cost so don't really meet your definition.
SNG administered to high risk patients / breathless patients/ those at a later stage of infection out of the hospital setting is very feasible. It is often discussed the cost of a hospital stay (even in the UK) and it quickly becomes cost effective when targeted to those most likely to need it.
Likewise I'm sure they will stop giving the Merk/ Pfizer pills to all and sundry because they aren't free !
Once SNG is out there; as you say in hospital, discharging to home will be the first application, I doubt it will be long before it's given to those at risk of being admitted to keep them out of hospital.
Yes it ain't cheap but if P2 is replicated the price will not be an issue.
