Despite the RNS it would be very disappointing if we fail to get a slice of the Covid funding pie. Guess it shows confidence in the science...now let's convince those who hold the purse strings.

Hi Tf, all good thanks. Shedding the pounds in the coveralls so I'm almost my ideal body weight. Admissions are certainly easing. Our hospital had a 25 step Covid19 surge plan each step increasing ltu beds. We have only reached stage 5 and I don't think we currently need to expand further. Has the curve flattened..... Hope so. Health care workers are patients and I note a report out in the Health Care Journal ,115 NHS staff have died. Reassuring to read statistical analysis shows we are at no greater risk of death than the general public, even ITU workers in the midst of areasole generating procedures. I'll post a link if I can find it on my phone. Statistics though....humm.

Sit in any NHS canteen and close your eyes. You will find most accents are European or foreign. The vast majority of NHS nursing , specialist and junior medical staff ( and almost 100 percent of domestic staff) are from Europe, mainly Portugal and Spain with Greece and Italy following at some distance. All are economic migrants. Take a moment and think about the nationality of the NHS staff who has treated you or a loved one recently. This is not about Brexit, the Government has given robust assurance in relation to settled status etc. It's about the UK verses the economy of Europe mainly Spain and Portugal. Who will offer the most financial assurance when the economy starts to recover. Personally I'd say the UK... so there's some hope... but if Europe pump whatever money they have into their health system and support this with wages it will be an entirely different situation , the economic migrants will flock home. The second worry is old fashioned "home sickness" following this global crisis. Family and national ties may prove stronger than financial security. Many have said they will return home when this is all over for no other reason than they feel a need to be with their families again. A third worry is the demographic age of the native UK NHS staff. Look at the age of UK nurses... Many are near retirement. When the gov promised 50000 extra nurses, 30000 of these were counted as "retained" staff. Staff who would stay due to improved job satisfaction, increased training and better working conditions. The way staff feel they have been treated by individual hospitals and let down over PPE by the Government there is almost zero chance of retaining these staff . This is the overwhelming feedback at the moment. Public and company support has been overwhelming but the resentment towards individual hospital management, the Government , and Public Health England especially negates this positivity. Many NHS staff will walk away once this is over. If the economic migrants follow, the NHS will be fatally wounded.

Hi C7, firstly I would like state that I claim no moral high ground working in the NHS, everyone's opinion is valid but that comment did upset me. There have been so many failings that could have been mitigated... Would any other government been any different, most likely not. I'm just so cross and angry at the moment about everything. The PPE equipment has been a disaster. We were not the first country to be affected and we should have already learnt the lessons. The PPE recommendations should have been robust from the beginning, not escalated in dribs and drabs. This caused NHS staff to be exposed and caused deaths. Every UK health care worker in critical care knew what prior countries had instigated, yet we were being initially told only to wear minimal protection. This caused mistrust against the gov from the off and mistrust in public health England. The latest PPE reusable guidance coming the day after the gov promised the care sector would be provided with PPE was no surprise. The gov could not even supply hospitals with enough let alone extend to care hones....so why not be honest in the first place and say the only way to provide equipment to everyone is to recycle. I was given a personal reusable mask in 2009 which only needs the fillers changed every 2-5 years, despite seeing these used in S Korea/ Italy and Germany the NHS told staff not to use them "we can't be sure they provide protection". We can now use them... but I have used countless disposable masks in the meantime.
It's not only the gov to blame it's the NHS management. Even when BJ was telling everyone to stay at home for 7/14 days individual hospitals were sending emails to staff "we need further guidance" .Staff were coming to work symptomatic. This is a fact. The gov should have told NHS management they HAVE to apply the same rules for NHS staff . Hospitals were not prepared for the level of sickness and tried to mitigate only giving staff "permission" to self isolate after they had "studied"the gov "advice". I have have heard this from many in the sector working in multiple hospitals. There is a saying in the NHS" poor managers and staff don't get sacked they get promoted. " This is so true. They all rotate jobs between themselves so no one is held to account and the same incompetentence continues. It's the front line staff who suffer. My initial observation about non white British being disproportionately affected has certainly played out nationally as this has developed.
As I said, it's the poor bloody staff who suffer irrespective of which gov is in power.

Just finished a stint. The Covid wards are currently taking the full brunt of patients and deaths. For those of you who are interested the latest detailed statistical info on patient survival in ITU remains at 50 percent. So much more detailed info is in the report. If you are healthy and not over weight it's a "comforting" report. A personal observation is the majority of the ITU admissions seam to be non white British.

I have such admiration for my colleagues but I have turned into a bit of a "radical communist " supporting workers rights. Management of the NHS is appalling. In a recent email to frontline staff who are working so hard under terrible circumstances and conditions was this sentence:. " we (the management)need to pace ourselves during this very difficult and stressful time, to this end we are reducing to a four day week. "
I'm staying six months to see this out ...then I will leave the NHS for good.

Preparation is racing ahead . A great deal has been said about face masks. The problem is one size doesn't fit all. In the ITU I work we require at least 5 different types to fit different shaped faces , for example Phillipino staff have delicate features and mostly need one type ....totally different to say a burly "fat UK guy ! In normal times you only stock afew boxes of different types as the need is not there . These are running out fast... compromising staff . The NHS gas been running like a "business" for many years now and we are encouraged to have only a minimal stock of items at any one time . This applies to equipment, drugs etc etc . Every hospital is now placing emergency orders and the system can't cope.
Ventilators...the prototype machines I see on the web being produced would do a great basic job ...but that is the problem...too basic. Very simple lesson for you guys : Take a deep breath in. That is your inspired Tidal Volume . Times this by your average breathing rate per minute = your inspired Minute Volume . Tidal volume (7mls per kg on average )..say 700 X respiratory rate of say 14=minute volume of 9800 or 9.8 liters per minute. This Miniute volume allows the oxygen to enter your lungs and carbon dioxide to be expired out. A basic prototype ventilator could achieve this. However it's SO MUCH more complicated. Covid patients get a respiratory syndrome called ARDS (adult respiratory distress syndrome) . This affects lung compliance ( ie how stiff your lungs become), fluid "shifts"into the lung making oxygen/ c02 exchange difficult. To treat this the ventilator needs to time both the inspired and expired breath to a set ratio allowing the oxygen and carbon dioxide time to "exchange". The vent needs to stop the exhalled breath from complete expiration....this "holds open " the lung to stop collapse of the delicate alveoli where the oxygen/co,2 exchange happens. If this fails the alveoli collapse and you loose lung function. Modern vents are very complex machines and basic ventilators won't cope with covid 19. The death rate once in ITU is 50 percent after 3 days... even on the complex vents.
Don't want to spread alarm but I hope a reality check will make you self isolate and encourage others to do so .

C7, My friend, where do I start. Preparations are being made for the mass influx and it's like waiting for the tusanami to hit. There has been a total breakdown in trust between the staff towards management and the NHS, not just where I work...it's the same in EVERY ITU and hospital from what I hear. After BJ et al give clear guidance the hospital trusts roll out their OWN guidance to staff ,often contrary to common sense and gov advice. They are doing this to try and minimise sickness but are destroying good will and putting lives at risk. Example : telling pregnant staff they need to work as "the guidelines from the government are not clear" and you don't need to self isolate if family members are sick "come to work". The latest is "you do not need to wear protective measures if a patient is a contact of another patient who has just tested positive " . Wards are so crowded how do they think there will have been no spread to other patients in the same crowded bay - the very staff they have instructed not to use protection will spread the virus to the most vulnerable patients. It's criminal.

UK death rate for covid +ve patients admitted to Critical Care is currently 50 percent , average duration of stay, 3 days. It's very grim on the front line. Remember.....only the peak of the pyramid reaches ITU .....but it's certainly a place you want to avoid. Be careful guys.

C7, it's now on the news . NHS England to postpone all non urgent surgery from the 15 th of April.

Elective surgery cancelled from 15 April . Well that's it I'm afraid. In reference to my last post the odds are now stacked impossibility against you. Elective should have been stopped at least 2 weeks ago.
